Clinical Care Manager – Northern Beaches

Welfare Recruitment Solutions has partnered with a highly reputable and respected NFP organisation to assist in recruiting a Registered Nurse for a senior community-based clinical role servicing Sydney’s Northern Beaches.
  
The opportunity further offers

  • A local role offering a highly flexible arrangement travelling to and from assessments across the week from home. Flexible start/finish times are also on offer.
  • Full-time permanent role working regular Monday to Friday hours where you can leave behind on-call, shift and weekend work

The Position

In this lead role you will oversee clinical care of HCP and transitional care clients travelling to their homes within the Upper Hunter area. You will be responsible for clinical assessments and ongoing review of their care plans aligned to goals and will lead, mentor and support Enrolled Nurses.  You will also work in conjunction with Care Managers/Co-ordinators assisting in the development of the care team.
  
Ideally you will have worked in community care nursing prior however very open to Registered Nurses who may approach this role from residential aged care or perhaps an acute hospital setting where you have spent time developing your clinical skills. Ideally you will have good knowledge and experience around wound care management.
  
The Person

  • AHPRA Registered Nurse
  • Current clinical experience ideally with good knowledge and experience around wound care management
  • Passionate about delivering quality and person-centred care service and possess natural leadership skills
  • Prior experience and knowledge in community nursing will be very highly regarded however further mentoring and support will be provided if you are new to HCP model.

  
This position presents a wonderful opportunity to step outside of residential care or the acute hospital setting and work within your local community in a senior clinical role. Dementia Care Specialists – Melbourne

The Opportunity

Welfare Recruitment Solutions is assisting with the recruitment of Dementia Care Specialists across Melbourne to provide face to face assessment services with persons living with dementia and their families.

  
The Role
  
You understand that dementia care is a person-centred approach presenting complex and unique challenges and through your strong experience and knowledge in dementia you will provide assessment services in line with best practice care.

Through this knowledge you will provide a face to face assessment service with persons living with dementia and their families in identifying factors that might be contributing to concerning cognitive behavioural changes.  
  
This is a great opportunity to guide, educate and support families and clinicians through your recommendations and follow-up/case manage to ensure that the strategies you recommend are successful.
  
 
The Candidate

To be considered for this position you will need to demonstrate:

  • Prior assessment experience either in a senior nursing, occupational therapy, speech pathology, or physiotherapy role within aged care working very closely with persons living with dementia.  You may be working in residential care, be community based, working for government health service or in an acute setting, GEM Unit/Older Persons Mental Health where you have been involved in assessing BPSD.  Alternatively, you might be working as an ACAT Assessor or as a Senior Diversional Therapist where you have gained strong knowledge and experience across different dementia diagnoses and understanding of the complex needs of dementia care.
  • Additional dementia behavioural management training or currently studying/completed Post Grad qualifications in Dementia Care/Gerontology/Ageing will be highly regarded.

This is a full time permanent positions based in Mascot with flexibility to work from home and 1-2 days/week in the office. We’d also consider a 4 day/week position.
 
Role
As the successful candidate your role includes addressing the specific local needs of people in the Metro Sydney region in relation to care finder support and encompasses the transition of the Assistance with Care and Housing (ACH) program to the care finder program. You will increase awareness in the local primary health care workforce of the availability of care finder supports, and help to improve integration between the health and aged care systems at a local level.

In order be considered for this position you must have the following:

  • Relevant qualifications in health related field
  • Demonstrated experience in stakeholder engagement and consultation ideally in the primary health sector
  • Experience in developing, implementing and evaluating population health programs
  • Excellent written and verbal communication skills
