Role

Your role will be to provide a variety of support to clients at the service location, and to assist in the entry and assessment of new clients, including accompanying children or young persons who access the service. You will provide a child safe, child friendly environment that is culturally affirming and promote the self-esteem of all children and young people who enter the service. You will be required to set up appropriately aged programs for the children, and playgroup activities involving the mother/carer and provide case management in consultation with the mother.

Candidate

  • Minimum Diploma qualifications in social work or related field.
  • Case management experience (specifically in child related or DFV roles)
  • Ability to maintain up to date case notes and work with other services to provide wrap around support
  • Drivers licence.

Relevant Diploma qualifications with extensive experience considered.

Further Info

In return, you will be offered an hourly rate of $45-50ph + Super. This is inclusive of a casual loading.

Ideally you will have worked in community care nursing prior however very open to Registered Nurses who may approach this role from residential aged care or perhaps an acute hospital setting where you have spent time developing your clinical skills.  Ideally you will have good knowledge and experience around wound care management.
  
The Person

  • AHPRA Registered Nurse
  • Current clinical experience ideally with good knowledge and experience around wound care management
  • Passionate about delivering quality and person-centred care service and possess natural leadership skills
  • Prior experience and knowledge in community nursing will be very highly regarded however further mentoring and support will be provided if you are new to HCP model.

The Client:
Our client is looking for a DV specialist, do you have experience helping women escape domestic violence? Welfare Recruitment Solutions have been approached by an industry leading not-for-profit organisation to assist in the recruitment of a case manager for their domestic violence crisis service for vulnerable women, on a 2 month contract which could lead to a permanent position. 

The Case Manager provides family violence risk and needs assessment, safety planning, crisis support and short-term case management to all incoming referrals for victim-survivors, referred by the DV Line, Link2home, and other specified referral agencies.

In this position you will be working within a team supporting women in a single women’s temporary accommodation.

You will be required to travel to outreach appointments.
 
The Successful Candidate: To be successful in this role, you will have the following:

  • Experience working with women and children escaping domestic and family violence
  • Experience in case management and in a domestic violence service women’s support service
  • Experience in needs assessment, safety planning, case coordination and advocacy for service users and in individualised outcomes- focused service delivery
  • Relevant tertiary qualifications in social work, community services or equivalent
